Resumindo temos 3 mecanismos de dor: nociceptiva, neuropática e nociplástica. .
• Nociceptiva: Devido a ativação de nociceptores (Inflamação, irritante mecânico, lesão). Ex: um corte; queimadura; Osteoartrite, torção de tornozelo, artrite reumatóide). .
• Nociplástica: Devido a um distúrbio no processamento central da dor (Aumento da excitabilidade, diminuição da inibição). Ex: Fibromialgia, Distúrbio Temporomandibular, Dor Lombar Não Específica).
•Neuropática: Devido a uma lesão ou doença no sistema somatossensorial. Ex: Neuropatia Diabética, Síndrome do Túnel do Carpo, Síndrome da Dor Complexa Regional. .
Alem desses mecanismos biológicos, os fatores psicossociais NUNCA devem ser negligenciados na avaliação de um paciente com dor.
Fatores como ansiedade, medo, estresse, depressão, relacionamentos com familiares, amigos e cônjuges, entre outros podem interferir diretamente na percepção de dor do paciente.
Portanto, muito importante entendermos o que é DOR e seus mecanismo, pois, a partir da avaliação também da DOR, podemos traçar estratégias para o alívio do quadro clínico.

Today marks #WorldDigestiveHealthDay . A day in which I reflect on how far I have come with my own personal digestive health story. I remember hearing the phrase A LOT as a kid. In high school my rugby team would say it to pump ourselves up before a game. I heard it in the gym to push ourselves harder in fitness. But what exactly is pain? And what does it do?
Now I still think you should get some exercise daily and find your limits. But what if we explored the meaning behind your pain?
👉Pain is a signal that your body sends to your brain to tell you that you need to reduce your activity so that it can heal. It doesn’t mean stop moving altogether.
🔥The inflammatory process is also our healing process. Whats that again?
Yes, the same inflammation we know that brings the pain, heat and redness is bringing blood flow to the area to breakdown the old or damaged tissues and bringing in the calvary to rebuild new tissue!
We can have acute (immediate) or chronic (ongoing) pain.
Acute pain is the pain after we just did some damage. The signal fires and we find ourselves with a rush of fluid (thats swelling), new blood flow will bring the heat and redness. Think of this as internal bracing. It is also similar to the process called cupping. For those of you that have tried cupping know sometimes bringing fluid to the area is just what the doctor ordered! This may be true when we have chronic pain.
Personally, after spending sometime in India and hearing about ayurveda, I think about pain differently. If it is flaming hot (acute) I think FIRE🔥Your immediate response might be ICE. But instead of icing which may restrict fluid flow- what if we pumped the fluid to keep it circulating and to flush it out. Much like a throwing water on a fire during camping.
This is the idea behind osteopathy too. Osteos use movements to improve circulation throughout the whole body (not just at the site of injury). Because no part of us is ever truly isolated.

Low back ever seize up while attempting to pick something up? Say a deadlift? Or woke up one morning with debilitating low back pain? WELL, it just happened to me...
Goal for rehab at this point is pain management and to maintain the mobility that we have. Try out these exercises and make sure to work within 𝐏𝐀𝐈𝐍-𝐅𝐑𝐄𝐄 ranges for these exercises.
1️⃣ 𝐙-𝐋𝐢𝐞 (10-15 mins every hour or as needed)
Elevate your feet onto a chair/couch/ottoman. Position your knees so that it is slightly past your hips (your legs should make a Z). You can also place a pillow directly underneath your pelvis to provide a bit more traction/decompression of your lumbar spine. While in this position, work on deep breathing, mindfulness and meditation.
• Note: This position should relieve your pain and not feel uncomfortable!
2️⃣ 𝐒𝐢𝐧𝐠𝐥𝐞 𝐊𝐧𝐞𝐞 𝐭𝐨 𝐂𝐡𝐞𝐬𝐭 (10x each leg 3x day)
Start on your back with both knees bent. Slowly bring your knee up towards your chest. Engage your core and slowly lower the leg back down.
3️⃣ 𝐇𝐨𝐨𝐤𝐥𝐲𝐢𝐧𝐠 𝐑𝐨𝐭𝐚𝐭𝐢𝐨𝐧 (10x each direction 3x day)
Start on your back with both kness bent (hooklying position). Have your feet and knees touching. Engage your core and slowly bring your knees side to side.
4️⃣ 𝐏𝐫𝐨𝐧𝐞 𝐄𝐱𝐭𝐞𝐧𝐬𝐢𝐨𝐧 𝐨𝐧 𝐅𝐨𝐫𝐞𝐚𝐫𝐦𝐬 (10x every hour)
Start on your stomach with your hands underneath your shoulders. While keeping your low back, gluts and legs relaxed, Slowly lift your chest off the ground with forearms. Make sure that your low back, gluts and legs are relaxed AND your hips remain on the ground throughout the whole movement.
• NOTE: if you notice that the pain is WORSE after you complete a set, STOP. If you notice that the pain is the same or getting better, then you are okay to continue.

One of my favourite topics.. Why lower back can be anyone's favourite topics????? Well, honestly.. It is much nicer to talk about travels or pets, or food but lower back pain is so common and so related to our life that it is normal that people talk about it.
I have to say I know about lower back pain quite a lot.. Not only because of my profession but because I have experienced it myself...And it wasn't a back stiffness that some people may have from time to time after long time sitting... I had a massive episode of back pain with sciatica-like symptoms that lasted for almost 2 months. If I have to give it a number out of 10, it would be 10..This story thaught me few important things about lower back pain..
1. Lower back pain can be excruciating and debilitating
2. Medics often don't understand how painful it is and make you feel like you are wasting their time with a back pain that usually goes away on its own..I guess they haven't experienced lower back pain themselves.
3. Lower back pain can start innocently as an achy muscular pain from prolonged sitting, incorrect posture and many others, but if neglected and the cause of the problem is not removed it can lead to much more painful episodes in the future.
4. Painkillers are good but only for a short while..And you may need something really really strong..
5. 💢 Low back pain is up there as the one of the most common issues among... well really everyone.
🙅♂️ Low back pain isn't just an older adult injury. It is not uncommon for active individuals in their 20s and 30s to report having experienced low back pain. Older individuals tend to experience it more often (chronic) and intensely due to the natural aging process.
🧠 What is being found is that a phenomenon known as Arthrogenic Muscle Inhibition (AMI) may be involved in many cases of chronic low back pain. AMI is essentially an injury to a joint that results in neural inhibition of an uninjured muscle surrounding that joint. This inhibition and immobility can lead to atrophy of the musculature.
🦵 If you have experience with ACL injury you probably have some idea of what AMI is. What is commonly seen in ACL injury is inhibition and atrophy of the the quadriceps muscle. Why? The answer isn't super clear, but the thought is that the associated inflammation with injury may be involved in disrupting neural pathways.
💡 In the low back, multifidus is the muscle of interest. For those of you who have never heard of or do not know much about the multifidi, it is a deep back muscle and functions as one of the main stabilizers of the lumbar spine and some stability of the SI joint. In cases of chronic low back pain, atrophy of the multifidus is apparent, which can lead to instability and increased chance of injury in the low back.
My two cents: Be thoughtful and intentional with your movements. If you experience low back pain, don't just shrug it off and think, "this is apart of the process, so just deal with it". You need to make adjustments.
If you are young, active and relatively healthy, recognize that these acute bouts of back pain are signals. And it is a possibility that sustaining many of these acute back injuries could make you more likely to develop chronic back injury later in life.

I have no idea why we brush over some injuries as if it’s not a big deal. 🤷♀️
Ankle injuries are one of these under-estimated traumas for sure!
And the impact unfortunately is the whole body MUST adapt to compensate for the changed walking pattern as we carefully baby the injury. Picture an ankle sprain (so good for this thought process if you’ve had one). For the case of this example we are going to ONLY talk about sprains on the outside of the ankle – the common type (lateral inversion injuries).
Stand up – have a feel where you weight falls in the sole of that foot.
Do you notice more pressure rolling inwards?
Think about the psychology of acute pain on the outside of the ankle from ligaments being stretched.
What will be the obvious body reaction? Shorten that area, shorten the ligaments and hold the bones close together, like an ankle cuddle 🤗. You will adapt to AVOID this injured space OPENING up. You are likely to feel the heel bone wanting to roll inwards, flattening the arch, with a GOAL to stay right away from any potential to sprain that ankle the same way.
Unless you consciously re-instate lengthening & experiencing that outside ankle area open safely in your rehab, you will likely avoid opening that ankle space ever again...
YES EVER AGAIN! You will be left with not just a funky ankle but a body under stress that will protect it forever.
IMPACT – you will avoid using your foot the way it’s designed, to arch & flatten, for the rest of your days & the rest of your body will cop it.
That’s how injuries work. We do something, then brain says “Oh Sh*t” and unless the traumatised area is specifically re-educated in it’s designed movement, that’s your lot, thanks for playing, too bad so sad. ☹️
So please think about the psychology of pain, it guides an enormous amount of understanding in what we avoid after injury, providing answers as to what movements to work on & re-experience. 🙂

Uma das frases que mais escuto dos pacientes com dor crônica na coluna é “minha dor vem da minha hérnia, minha ressonância mostra certinho” e na maioria das vezes quando pergunto, há quanto tempo eles sentem essa “dor da hérnia”, ou há quanto tempo atrás foi feita a ressonância, eles referem 2, 5, 10, 15 anos. E quando apresento essa estatística para eles, os mesmos ficam sem resposta, ou relutam e falam que eles estão nos 33,33% (crenças enraizadas). As vezes é difícil convencer que eles não sentem “dor da hérnia”, mesmo com todos os testes neurológicos negativos. O entendimento da não-especificidade da dor crônica por parte dos pacientes é difícil, pois todos fomos condicionados a pensar que dor só ocorre com lesão, e isso muita vezes se torna um desafio para descomplicarmos ao paciente.
Como resolver isso? Educação em Neurociências da Dor, muita conversa, e dependendo do nível de enraizamento das crenças, a indicação para um psicólogo!

The major goals of rehabilitation of the ACL-injured knee:
-Gain good functional stability
-Repair muscle strength
-Reach the best possible functional level
-Decrease the risk for re-injury
Closed kinetic chain exercises (CKC) and Open kinetic chain exercises (OKC) play an important role in regaining muscle (quadriceps, hamstrings) strength and knee stability.
CKC exercises have become more popular than OKC exercises in ACL rehabilitation. Clinicians believe that CKC exercises are safer than OKC exercises because they place less strain on the ACL graft. Besides, they also believe that CKC exercises are more functional and equally effective as OKC exercises.
Exercises should encourage range of movement, strengthening of the quadriceps and hamstrings, and proprioception. Consider integrating any of these exercises into a rehabilitation programme at this stage as appropriate for the client:

5️⃣Glut med work in side lying
6️⃣Glut exercises in prone
7️⃣Knee flexion in prone (gentle kicking exercises)

I threw my back out a week ago.
Technically it was my back and hip. But when I'd popped my subluxy, traitorous hip back into the joint.. the back was still an issue. A loud one.
I'm still in extra pain. Enough so that today my kids were hyper focused on me. "Are you sad?" one asked me. "Do you need us to be quieter?" another asked, an hour later. "No, sweetheart," I said, over and over, "Just hurting. Just hurting again today." I am grumpy about it, now. The first few days of new pain are novel. You're in survival mode, you and the pain. After a week, you're familiar enough to be bitter with each other. The claws come out. You're not so sure it's going to pass. Especially for those of us with chronic pain, the temptation to catastrophize starts scratching away.
A coworker reminded me, when I apologized for missing another meeting of a club we co-chair, that it's okay to hate it. "I know you'll be okay. You'll make it. But it's okay to bitch sometimes." Here is me, one time when my body let me do things I liked. I had just gotten my disability access pass. I was going into the woods to see some Very Big Trees.

Yesterday at our work conference we spoke about resilience and courage and who inspires us. At this moment I was struggling.. I didn’t sleep because of the pain. I had to sit in a group of random people whilst being in pain and I put up all road blocks and said to myself ✋🏼”I don’t want to be here”. ✋🏼I just wanted to run!!! I could feel a panic attack coming on,my chest was so tight, my tears were about to burst and I had to stop myself.
What thoughts were going on in my head to make me loose it.
1. I was in so much pain I didn’t want to be there
2. Why should I have to work when I feel like this?
3. I have to dig deep each day and find courage to get to work and going through all the shit I’ve gone through makes me a professional at resilience 😂 so I don’t want to talk about this shit. (Very aggressive thought I know)
But it really made me think. A lot of the people in my group didn’t have pain or illness in their lives and they admired people for certain things that to be honest didn’t interest me.
I try my best to not compare myself to anyone else. I don’t try and BE anyone else. All I do each day is focus on the day. I want to do my BEST today, to be HAPPY to maybe try something new but to just be present and at peace. And that may seem so generic but that is what I focus on with chronic pain.
Chronic pain is invisible, people don’t see your struggles and even if they do sometimes they just can’t understand how you could be in pain. I’m learning not to care how people persevere me but to focus on how I can be a voice for people suffering with chronic pain.

Before we talk about Chronic pain, let's understand Acute pain first.
We can all agree that pain is a universal human experience however, it differs from one person to the other.
No matter how your pain feels or how long you've had it for, it's caused by the brain!
So how does the brain do that?
There are sensory neurons in our body called Nociceptors (located on our skin and organs, except for the brain) that detect harmful changes in our body.
Let's say you've hurt your big toe. Nociceptors located on the skin of the big toe will carry the message up the leg, through the spinal cord, up to the brainstem and then it relays the information to different regions of the brain.
The communication between these regions will give rise to the feeling pain, will localise it and will send instructions back to the body to react accordingly.
Pain makes us miserable but it draws our attention to injury so we can take care of it.
Can you imagine being bitten by a poisonous snake and not knowing about it before it's too late?

A New Birth, A New Journey🙏🏻
IT’S HERE!!!!!!!! God has birthed something AMAZING from our journey of pain and loss. 2 years ago our lives changed forever when Paul was hit by a car while riding his bicycle. A Friday Flashback of the Kaiser Sports Fellowship Crew 2018.
For all the PT students out there or clinicians who may be wondering is there more to learn? ABSOLUTELY YES. .
A great way to elevate your clinical skills and help elevate the field in general is by really consistently working on your craft and eventually teaching/mentoring the clinicians coming into the field behind you. Residency was a great place to start, fellowship really kicked it up a hole notch! There is no way I’d be the therapist I am right now without my mentors ! (And I KNOW there is still so much to be done to really perfect the craft).
There is ALWAYS more to do in order to truly become an “expert” . .
As newer-clinicians - If you find yourself thinking - “I’ve got this” let’s be real... hmm you probably don’t —-because how can you know what you DONT know?! .
This is my go-to, all-natural blend when I'm experiencing some intense discomfort, or know someone else who is. And I just started suggesting the addition of Copaiba for extra support.
